Hajimemashite minna-san. Watashi no namae wa Midorikawa Akisa desu. Akisa tomoshimasu. Juunana sai desu. Douzo yoroshiku onegaishimasu.

hajimemashite
is used when meeting a person for the first time. It can be:
1. How do you do?
2. Nice to meet you

I/my/me/mine, it's up to the particle in the sentence.

Akisa Midorikawa
ともします

you can call me.../please call me...

yoroshiku onegaishimasu
it has many different meanings, depending on the situation. In this case (introducing) it acts as a greeting, similar to:
1. nice to meet you
2. be nice to me
So this is the full meaning of my Japanese Introduction:
How do you do everyone? My name is Akisa Midorikawa. You can call me Akisa. I am 17 years old. Please be nice to me.
